Schedule Second Phone Interview:

Step 4

Intake Interview:

  • Please bring your Photo ID, Social Security card, and any personal items you’ll need. (listed in the "Things To Know" packet) The final interview will determine whether you are admitted into the program or if the team recommends something different.

  • This is a real commitment to your recovery journey.
